Full-time, Part-time and Per Diem schedules are available.

Responsible for the oversight of medical care including medication regimens, medical/psychiatric conditions and routine follow-up. Provides oversight of medication administration at all assigned sites; provides nursing coverage, as needed, at all assigned sites. Trains site staff in the administration of medication, all applicable nursing assigned tasks, and ensures compliance with OPWDD regulations.

Responsibilities
  • Models and promotes Person Centered Planning, considering the dreams and goals of individuals being served.
  • Monitors each Individual’s healthcare clinical status and treatment response. Develops Plans of Care specific to each individual’s needs.
  • Conducts individual-specific medication reviews weekly to ensure the proper administration of medication targeting accuracy of administration and compliance with physician orders. Consultation with agency psychologist as needed.
  • Provides documentation of weekly medication reviews to the Nursing Services Administrator.
  • Completes data entry into Quickmar and Therap. Assists with maintaining the database to ensure individual files are updated and current.
  • Completes Clinical note, weekly, for every individual assigned.
  • Completes clear guidance within the Nurse Communication record. Ensures appropriate follow up occurs as requested.
  • Evaluates each individual living in a certified residential setting for the ability to self-administer medications within 60 days of admission and at least annually thereafter.
  • Coordinates clinical meeting with psychologist, and clinical team members monthly or more frequently if needed.
  • Works with multidisciplinary team in establishing goals and evaluating outcomes for each individual.
  • Monitors the scheduling and attendance of all healthcare appointments.
  • Provides training to Agency Staff in the medication administration approved training curriculum, including on-site medication pours and required documentation. Approves staff for certification and makes recommendation for removal of employee certification, when necessary.
  • Provides supervisory guidance to staff as needed while on site and while on-call.
  • Utilizes Health-e-Link and Pharmacy Portal, as needed, to access necessary medical records for quality continuum of care to individuals receiving services.
  • Responsible to ensure that all staff are competent in Nursing assigned tasks. Responsible for the training of all related tasks to ensure competency.
  • Provides medical and emergency treatment as needed. Administers medications and injections to individuals as needed. Coordinates with the Pharmacy, Service Coordinators, and Medical Providers to ensure that adequate supplies of medications are on hand.
  • Responsible for rotational on-call twenty-four (24) hours per day for all individuals’ medical needs including consultation in medical emergency situations.
  • Attends staff meetings, periodically, to ensure comprehensive medical services is provided to all individuals.
Education and Experience
  • Licensed and registered by New York State Department of Education in Nursing.
  • Experience in OPWDD certified residential/day habilitation setting preferred.
Skills, Knowledge and Abilities
  • Knowledge of nursing, medical supplies, and medical terminology.
  • Good written and oral communication skills.
  • Excellent training, facilitation, mentoring and interpersonal skills.
  • Working knowledge of industry regulations and procedures.
  • Must be timely and prompt in performing tasks, as well as demonstrate the ability to exercise independent judgment and to meet deadlines.
  • Knowledge of person centered planning and approaches.
  • Basic computer knowledge and database management skills.
  • The ability to maintain orderly files and records.
Certifications
  • Must hold and maintain NYS Licensing requirements throughout employment and provide updates as required.
Physical and Mental Requirements
  • Seeing, hearing, walking, sitting, standing, stooping, stretching and driving.
  • Exercising independent judgment, concentrating, thinking, communicating, interacting with others, managing multiple tasks and coping with stressful situations and deadlines.
  • Staying awake and alert while on duty.
Working Conditions
  • Full time schedule to be coordinated with supervisor, expected 40 hours per week.
  • Part time schedule to be coordinated with supervisor.
  • Relief schedule to be coordinated with supervisor;
    Relief positions must work a minimum of 12 hours per month. Training hours do not count towards required hours.
  • Will usually work normal business hours. May be required to address emergency situations during evening or weekend hours.
  • Frequent local travel required. Mileage reimbursement provided.
