This position is responsible for assessing psychosocial needs of patients and families, providing professional counseling, case management, and discharge planning services. Services are provided in accordance with professional standards and in collaboration with the multidisciplinary health care team to assist patients and families in utilizing medical care constructively and to achieve optimal functioning in all patient care settings.

Master’s Social Work or related field required. NYS driver’s license upon hire required. PASRR screen within 6 months required. NYS DOH PASRR screen certification required within six months of hire for all inpatient departments.

3 years of counseling experience required, excluding School Health Services departments.
