Job DescriptionThe incumbent is involved in a high level of community outreach & engagement, education and enrollment. Meet with families to explain the role of the Health Home and how it can assist the members and the families. Conduct intake interviews with members and families, including enrolling and/or referring members to other community services. Meet in with families in their homes, at clinics, in schools, in the emergency room and at community events.
Must have the ability to travel in the Western New York area. Work flexible and variable hours; including evenings and weekends. Process referral and enrollments for members up to the age of 21.
HS - High School Diploma or GED required. NYS Drivers License required upon hire. CANS NY 0-5, CANS NY 6-21 preferred.
Experience2 years of experience in direct sales, health/social services experience and/or education preferred.
