, we’re hiring a Floating RN Case Manager to support our growing service areas in the Greater Twin Cities. This full-time position offers variety, the consistency of 1:1 home care, and strong team support.
- Diverse experience: Support a range of clients and care teams across the metro
- High impact: Step in to stabilize caseloads and ensure consistent, high-quality care
- Autonomy + teamwork: Work independently while staying connected to a responsive clinical leadership team
- Incentive pay: Earn additional compensation for visits in support of fluctuating census or expanded territory needs
- Deliver skilled nursing care and perform comprehensive client assessments
- Develop and update individualized care plans
- Provide caregiver education and support to maintain quality standards
- Rotate into the on-call schedule
- Travel as needed to different areas of the Twin Cities metro to assist with coverage
- Active RN license in the State of Minnesota
- Reliable transportation and valid insurance (position involves regular travel)
- Strong clinical judgment and communication skills
- Experience in home health is preferred
