Altercare Bucyrus is seeking an LPN Alzheimer’s Program Coordinator for their specialized, secure unit. Deliver programming with and through your CNAs to enrich the lives of the residents. • Oversee and supervise all aspects of care and programming for dementia residents.
• Design and implement creative activities for dementia residents.
• Develop care plans and perform assessments.
• Conduct resident screenings and family interviews.

• Licensed Practical Nurse (LPN) with valid Ohio License.
• Experience in development and facilitation of activity programs preferred.
• Prior experience in a skilled nursing, long‑term care, or geriatric setting preferred.
• Knowledge of Alzheimer’s disease and related dementia appreciated.
