Responsibilities

Perform comprehensive assessments of hospice patients to evaluate clinical status and care needs

Develop, implement, and manage individualized care plans in collaboration with physicians and interdisciplinary team members

Conduct admissions, routine, PRN, and death visits as required

Continuously assess and reassess appropriate levels of care to ensure cost-effective and clinically appropriate services

Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and other care partners

Required Qualifications

License Requirements: WV or Compact RN license

Preferred

Skills:

Hospice and HCHB experience required; must be able to complete admissions, routine, PRN, and death visits

Associates or Bachelor's Degree in Nursing or related field

Ability to analyze complex medical information and make rational decisions

Case Management certification

BLS/CPR
