Hospice Registered Nurse

Location

Williams, Defiance, Fulton and Henry Counties.

As a Hospice Registered Nurse (RN) Case Manager at Ohioans Home Healthcare & Hospice, you will play a key role in an organization with a reputation for unsurpassed quality care, a close-knit, family-friendly culture, and a supportive management team as we begin to introduce hospice services to our Bryan area territory.

We will rely on you to assess, plan, and implement patient care; educate patients and families on comfort care processes, procedures, and medications; and address various patient needs.

  • Quick access to drug information, patient education and more
  • Animations, videos and illustrations of procedures
  • Updates on best practices
  • Online assigned tasks and training

As a member of our hospice registered nurse (RN) team, you will have the opportunity to build satisfying one-on-one relationships with patients and families by providing comfort care at the end of life.

• Current RN state licensure
• CPR certification
• Ability to travel for in-home patient visits
• Exceptional time-management skills
• Excellent interpersonal and customer service skills
• Hospice/case management experience preferred
• This position will include an on-call rotation.
