Home Health Nurse

Role Overview

The Home Health RN provides skilled nursing care to patients in their homes, coordinates care with interdisciplinary teams, and ensures high-quality, compliant patient care and documentation.

• Assess patients in stable and emergent situations
• Develop, implement, and evaluate Plans of Care (POC)
• Educate patients and families on care, medications, and safety
• Perform wound care, IV care, phlebotomy, catheter/drain care, and respiratory treatments
• Coordinate referrals and ensure payer compliance
• Supervise HHAs/LVNs as needed
• Participate in team meetings and on-call coverage when assigned

• Active Georgia RN license
• 2+ years Home Health experience (required)
• OASIS experience (required)
• Homecare Homebase (HCHB) experience (required)
• BLS/CPR certification
• Valid driver’s license, reliable transportation, and insurance
• Ability to travel locally

• CHPN certification
• BSN
• Patient Service Area: Glynn, Camden, Charlton, Brantley, Wayne, Long, and McIntosh Counties
• Productivity: 30 units/week
• Floating: Yes
