Job OverviewClient is looking for skilled Nurse Practitioners and Physician Assistants eager to make a difference in patients’ lives. The Advanced Practitioner is a key member of an integrated Care Team, which includes a Registered Nurse and a Social Worker. Patients often suffer from multiple serious diseases; you will help improve quality of life, slow the progression of kidney disease, and achieve positive health outcomes.
Using your skills, you deliver personalized, compassionate medical care to individuals with chronic illness or kidney disease. You will also care for patients, maintain accurate records, and schedule follow‑up appointments.
Roles and Responsibilities- Conduct assessments, including comprehensive annual wellness exams in patients’ homes or virtual environments.
- Counsel and educate patients and families about benefits and programs to improve health.
- Document chief complaint, diagnoses, medical/social history, examination, medications, allergies, assessment, and plan.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Complete all documentation and paperwork in a timely manner.
- Maintain quality of care standards as defined by the practice.
- Work remotely in the state of employment; attend periodic training/meetings outside the state.
- Deliver evidence‑based, timely care that reduces avoidable hospitalizations and maximizes quality of life.
- Prescribe medications, order tests, and collaborate with the client physician.
- Measure and report patient quality outcomes daily.
- Educate patients about preventive monitoring and assist scheduling of diagnostic testing.
- Assist patients with enrollment into educational videos.
- Participate in integrated care team meetings.
- Apply disease diagnosis and prevention knowledge to care plans.
- Follow up and evaluate patient status.
- Perform other duties as assigned.
- Active and unrestricted RN, NP, or PA license.
- Board certified (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work autonomously without direct supervision.
- Valid driver’s license, access to transportation, and car insurance.
- Proficiency with required medical instruments and equipment.
- Knowledge of computer‑based data management programs and medical record systems.
- Effective verbal and written communication skills with patients and partners.
- Understanding of HIPAA, OSHA, and other federal/state regulations.
- Minimum of 2 years of direct patient care.
- Experience conducting annual wellness or similar visits virtually or at home.
