Physical Therapist Home Health
Humana is seeking a Physical Therapist Home Health to provide personalized rehabilitation services within the patients’ homes. The successful candidate will work with physicians, PT assistants, and home health aides to develop and execute treatment plans that promote recovery and independence.
Responsibilities- Plan and administer physical therapy treatments to patients with injuries, illnesses and functional disabilities to reach optimal physical function.
- Screen patients’ physical strengths, assist physicians in evaluating level of function, document findings and develop measurable training objectives.
- Develop and implement conditioning/rehabilitation programs aligned with the physician’s Plan of Treatment and team goals; adjust treatment as needed to achieve results.
- Coordinate with physicians, home health aides, PT assistants, and other team members, provide written instructions and supervise staff performance.
- Accurately document care observations, interventions and evaluations; ensure timely submission of physician orders and treatment plans.
- Report patient progress to physicians, clinical manager and family; provide evaluation, treatment and discharge summaries.
- Degree from an accredited Physical Therapy Program approved by the APTA.
- Minimum of one year physical therapy experience preferred.
- Current unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
• $54.00 – $76.00 per visit/unit
• $85,400 – $117,500 per year base pay
