Evaluate patients within twenty-four (24) hours, or 1 business day of physician referral. If unable to complete evaluation in 24 hours, documentation in the medical record must clearly define the cause for delay.
Develop an effective treatment plan and obtain approval for services from the referring physician. Treat patients according to the physician's treatment plan. Assist the nursing department with training Restorative Aides. Supervise Physical Therapy Assistants in direct patient care and patient-related activities, following the state practice act.
Communicate with supervisors and other health team members regarding patient progress, problems, and plans. Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Engage in in-service training programs for other staff within the facility.
Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, adhering to PT Board State Practice Act and governmental and third-party payer requirements. Document treatment changes according to policy and procedures.
Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Participate in discharge planning.
Recommend and facilitate the ordering of necessary durable medical equipment to promote independence in mobility (e.g., braces, walkers, canes, elevated toilet seats, grab bars). Report any equipment problems to ensure proper maintenance.
Maintain positive, effective, and appropriate relationships with patients/residents, families, staff, and professional colleagues.
Supervisory RequirementsSupervise PT assistants, aides, and students.
Qualifications- Education and/or
Experience:
Bachelor's degree in Physical Therapy;
Master's or Doctorate preferred. - Proficient in computer skills, including programs like Casamba and Point Click Care.

Ability to read technical procedures, policy manuals, and effectively communicate with managers, coworkers, and families. - Mathematical
Skills:
Ability to apply fractions, percentages, ratios, and proportions. - Reasoning Ability:
Problem-solving skills and ability to interpret various instructions. - Certificates, Licenses, Registrations:
Licensed as a Physical Therapist in the state; must meet continuing education requirements and maintain licensure in good standing.
Requires standing, walking frequently; sitting occasionally; reaching, pushing, pulling frequently; talking and hearing frequently; tasting and smelling frequently; exerting force over 100 pounds occasionally, over 50 pounds frequently, and over 20 pounds constantly; occasional climbing, balancing, stooping, kneeling, crouching, or crawling; and good visual perception.
