Nurse Practitioner (Skilled Nursing) Major Responsibilities- Act as the Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians, serving as a day‑to‑day clinical leader while helping oversee the care of all residents.
- Work with the entire patient population—short and long‑term residents—overseeing their day‑to‑day care with nursing staff assistance and guidance from medical directors and attending physicians.
- Evaluate new‑admission visits.
- Coordinate discharge visits.
- Perform monthly stability visits for long‑term residents.
- Conduct evaluations for patients exhibiting changes in condition.
- Provide various types of care, including:
Advanced Care Planning; comprehensive health assessments, differential diagnosis, and prescribing treatments under protocol for acute and chronic illness and disease; prescription of all necessary medications and treatments; comprehensive physical examinations; development and implementation of patient management policies and procedures; facilitation of referrals to other healthcare professionals and medical facilities; communication with attending physicians and/or specialists regarding patient care.

- Salary range: $105,000 - $120,000+ commensurate with experience.
- Uncapped bonus opportunity.
- Advanced Practice Licensure (APN) with a Florida license.
- Active license and DEA number.
- Previous experience in nursing or other medical fields.
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast‑paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master’s degree required.
