Position SummaryThis position provides continuity of care, with the goal of permanent placement, for children through a family care management model that includes developing, expanding, accessing, and linking resources in the community to the needs of the child throughout the child’s experience in the system, while documenting progress.
What You Will Do- Strong verbal and written communication skills.
- Ability to document clearly and concisely.
- Assess, interpret, and provide care information for children and families.
- Work as part of a Family Care Team managing children (ages 0-17).
- Demonstrate a positive attitude, patience, respect, and cultural sensitivity in challenging environments.
- Personal vehicle use may be required.
- Serve as a mandatory reporter for child safety.
- Knowledge of community resources.
- Valid Florida driver’s license and agency insurability required.
- Punctuality and readiness to work.
- Comply with EEO and promote workplace dignity and respect.
- Collaborate effectively in team settings.
- Take guidance and direction from supervisors.
- Adhere to agency core values and mission.
- Proficient in Microsoft Suites.
- Contribute as part of the family care team.
- Provide case management services including assessment, planning, monitoring, and advocacy.
- Ensure completion of Medicaid assessments and case/treatment plans for children in foster care.
- Develop and maintain service/case plans with clear goals, objectives, and timelines.
- Review and update case plans regularly.
- Complete and share monthly progress reports.
- Provide educational support to families about child abuse and neglect.
- Maintain accurate records in electronic and paper systems.
- Ensure timely preparation of legal work per Florida Statute.
- Identify and monitor at-risk families, informing supervisors as needed.
- Refer children and families to community services for continuity of care.
- Treat children and families with respect and courtesy.
- Participate in staff development, team meetings, and client reviews.
- Build and maintain effective working relationships with team members, supervisors, community providers, and families.
- Complete additional tasks or projects assigned by management.
- Authorized to access protected health information.
Hernando County
RequirementsEducation & Experience: BSW/BA or MSW/MA in human services; direct experience in FC/Adoption preferred.
Physical Requirements: Ability to travel regionally on a regular basis and statewide as needed; ability to bend, lift, and carry 30 pounds.
All Family Care Managers — Probationary Proficiency- Completed car seat training.
- Completed pre-service training.
- Able to represent and discuss case progress in court independently.
- Able to complete accurate, comprehensive home visit notes.
- Proficient in timely documentation and navigation of the FSFN system.
- Prepare detailed and timely court documents.
- Consistently meet time frames for child visits, parent contact, and FFA/progress updates.
- Understand community resources and referral processes.
- Provide excellent customer service by responding to emails/voicemails within 24 hours.
- Use effective time management processes like lists, calendars, and tracking tools.
