Job  Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary

We have an exciting opportunity to join our team as a Social Worker - Outpatient Behavioral Health - Enhanced Treatment and Recovery Program. The Social Worker is assigned to see patients and provide a wide-array of clinical services including individual, group, family psychotherapy and case management both onsite as well as remotely from home or a designated location.

Job Responsibilities
  • Demonstrates knowledge and skills necessary to provide care based on physical psychosocial educational safety and age-related criteria pertinent to the patients served in his/her assigned area.
  • Performs thorough initial and ongoing assessments of patients at intake and/or during the course of treatment.
  • Demonstrates knowledge and skill required to provide accurate assessments and diagnosis.
  • Performs thorough treatment and crisis intervention.
  • Involves family and or/significant others when indicated in order to provide comprehensive care.
  • Requests consultation when necessary from treatment team psychiatrist or nurse practitioner.
  • Reports child abuse/neglect when indicated.
  • Maintains productivity
