We are actively seeking an experienced and compassionate Registered Nurse (RN) to serve as a Wound Care Nurse at our reputable Four Seasons Nursing and Rehabilitation Center in Brooklyn, NY. This is a full-time position with immediate availability, ideal for a clinically skilled nurse who is passionate about wound management and improving resident outcomes in a long-term care setting.
Key Responsibilities:Assess, plan, and implement wound care treatment for residents
Monitor and document wound healing and progress
Collaborate with physicians, interdisciplinary teams, and family members
Educate nursing staff on wound prevention and care best practices
Ensure compliance with facility policies, DOH regulations, and documentation standards
Participate in quality improvement initiatives related to skin and wound care
- Additional duties as assigned by job description, Administrator, DON, and Regional Support team
Current New York State RN license – required
Prior wound care certification or training - required
Experience in long-term care or subacute/rehab setting – preferred
Strong assessment, communication, and organizational skills
Knowledge of wound vacs, pressure injury prevention, and skin integrity protocols
Must pass background check and drug screening
