Registered Nurse (RN) Case Manager Position in Brooklyn!
Full time position!
Salary: $110,000 per year plus benefits!
Position Overview The RN Case Manager serves as the primary link between medical services, residential program staff, and external clinical providers. This role is responsible for overseeing comprehensive nursing care for individuals residing in ICF/IRA programs, ensuring all medical services are delivered in accordance with professional nursing standards and applicable state and federal regulations.
The RN Case Manager supervises LPNs, AMAP staff, and Direct Support Professionals in the performance of nursing tasks and is expected to have a working knowledge of OPWDD rules and regulations. Key Responsibilities
- Provide direct nursing care as prescribed by physicians or nurse practitioners
- Conduct ongoing health assessments and evaluations
- Maintain weekly interaction with individuals served
- Perform nursing assessments upon entry and quarterly thereafter
- Develop and maintain individualized nursing care plans
- Coordinate acute and ongoing healthcare services
- Monitor health status, labs, and treatment effectiveness
- Ensure accurate and timely medical documentation
- Train staff in nursing tasks, infection control, and preventive health
- Train staff in medication administration, tube feeding, and diabetic care
- Conduct annual clinical competency evaluations
- Maintain training documentation
- Utilize electronic health record systems
- Participate in Interdisciplinary Team meetings
- Perform other duties as assigned
• Active, unrestricted New York State Registered Nurse (RN) license
• Bachelor's degree preferred
• Minimum of three years of clinical experience preferred Experience & Skills
• Experience working with individuals with developmental disabilities preferred
• Supervisory experience preferred
• Strong communication and organizational skills
• Ability to work independently and as part of a team Technical Skills
• Proficiency in Microsoft Office Suite
