MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services includes home care, hospice and palliative care, rehabilitation and nursing care, and the MJHS Institute for Innovation and Palliative Care. We also offer Elderplan/Home First health plans for Medicare and dual-eligible individuals.

MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery, enabling recovery in the patient’s home with a comprehensive range of services supported by an integrated health system.

• Under the direction of a Coordinator of Care RN, perform home visits to patients based on skilled need.
• Implement the plan of care and evaluate the patient’s treatment.
• Document the visit electronically within 24-48 hours.
• Observe, measure, record, and report clinical data related to a patient’s health status.
• Collect, communicate, and document signs or symptoms outside established parameters indicating decline in health status.
• Under the direction of an RN, accurately reconcile and administer prescribed medications according to the physician’s plan of care.
• Under the direction of an RN, perform nursing treatments (e.g., urinary catheterization, sterile dressing changes, tracheal suctioning) as indicated in the physician’s plan of care.

• NY State LPN License required
• Minimum of one year Med/surg experience in a hospital or long-term care facility required
• Must have a driver’s license and own car
• Good computer skills required
