Overview
Senior UAS Assessor - Steuben County. Health Care Support is actively seeking a Senior UAS Assessor to support members in the Bronx or Brooklyn, NY. This organization is known for its commitment to high-quality, community-based care management and its focus on accurate, timely assessments to support long-term services and supports. In this role, the assessor will conduct comprehensive UAS-NY assessments, collaborate with care teams, and help ensure members receive appropriate, high-quality services—offering a meaningful position with direct impact on patient outcomes.
ScheduleMonday – Friday, 8:30 AM – 5:00 PM, No on call
Compensation$90,000 – $95,000 annually (DOE). Includes:
- 8% Annual Bonus
- Mileage & toll reimbursement
- Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes
- Develop and manage personalized long-term care plans while educating members and families on available services and benefits

- Active and clear NY State RN license
- 2+ years of recent experience as an RN
