Position Overview

SCOPE OF ROLE:
The ACT teams Psychiatric Nurse Practitioner partners with the multi-disciplinary team to deliver comprehensive mental health treatment to individuals who have a mental illness that seriously impairs their functioning in the community. Serving those whose needs have not been met by traditional clinic-based approaches, this innovative, recovery-oriented, ‘best practice’ intervention delivers treatment to people in their homes, shelters, parks and anywhere else they happen to be living, engaging them in a sustained relationship that leads to recovery.

ACT Teams are a mobile interdisciplinary team of professionals (psychiatry, nursing, social work, rehabilitation).

Must be registered as a NY State Medicaid provider. Must have a DEA# and an individual NPI #.

Responsibilities
  • Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements.
  • Review health assessments completed by the Program Nurse and provide clinical feedback.
  • Provide medication management including prescription refills and medication education to consumers.
  • Document patients  medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes.
  • Provide training to other team members on topics including medications and drug interactions.
  • Communicate and coordinate care with psychiatrists in the community, as needed.
  • Assist with psychiatric emergencies, refer residents to emergency care if needed.
  • Provide crisis intervention and address all emergency situations as appropriate.
  • Maintain up to date, accurate, complete, and timely consumer chart documentation.
  • Attend staff meetings, case conferences, group supervision, and trainings as needed.
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
  • Coordinates care with S:

    US nurse practitioners, medical director and administrative staff; closely coordinates with clinicians external to S:

    US as appropriate.
  • Utilizes best and evidence-based approaches to treatment consistent with S:

    US  organizational culture; health/mental health integration, rehabilitation and recovery, and wellness approaches figure prominently.
  • Perform other related duties as assigned.
Qualifications

REQUIRED EDUCATION AND EXPERIENCE

  • Licensed as a Registered Nurse in New York State and credentialed by the State to practice as an advanced Nurse Practitioner with a specialty in psychiatry. Must be registered as a NY State Medicaid provider. Must have a DEA# and an individual NPI #.
  • Maintain CPR/First Aid Certification.

PREFERRED QUALIFICATIONS & SKILLS

  • Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with severely and persistently mentally ill, substance use and homeless populations. Ability to work independently and as part of a team
