CompensationPay Range: $50.00-$55.00 per Hour
DescriptionWork with mental health professionals to assist clients transitioning from hospitals to the community.
Utilize the Critical Time Intervention model to ensure clients are linked with community providers.
Diagnose and Ntreat human responses to health problems through case management and counseling.
Provide supportive or restorative interventions and execute prescribed medical regimens.
Limited on‑call coverage required.
- Complete medical and communicable disease risk assessments within mandated time frames.
- Coordinate medication delivery and reception with pharmacy.
- Track and manage medications, including returns to pharmacy.
- Observe self‑administration of medications for detoxification as per protocols.
- Refer clients for necessary medical services.
- Conduct urine screenings according to program policy.
- Provide health education and consultation to clients individually or in groups.
- Valid NYS RN license.
- Knowledge of chemical dependency as a primary diagnosis.
- Excellent written and verbal communication skills.
- Experience in assessment and case management.
- Proficiency with generic office software tools.
- Ability to provide crisis intervention with interpersonal skills and maturity.
- Bilingual (Spanish/English) speaking preferred.
