Registered Nurse – Diabetes Care (Outpatient) | Brooklyn, NY Assignment Details

• Start Date: 02/24/2026

  • At least one evening per week required
  • Some Saturdays required
  • Two weekends per month

• Patient Population: Adult Diabetes Management
• Location: Brooklyn, NY (11203)

• License: Active New York State RN
• Education: BSN required
• Certification: Diabetes Education Certification required
• Experience:
  • Outpatient RN experience
  • Strong knowledge of diabetes management
  • Hands-on experience with CGMs and insulin pumps

• Provide comprehensive diabetes education and patient counseling
• Educate patients on insulin administration, CGMs, insulin pumps, and glucagon use
• Troubleshoot diabetes devices and coordinate with vendors
• Conduct glucose tolerance testing
• Support medication prior authorizations and insurance appeals
• Upload and manage patient device data for in-person and virtual visits
• Provide nutrition and exercise counseling
• Collaborate with endocrinologists, NPs, dietitians, and health coaches
• Support patients with diabetes distress and coordinate referrals as needed
