Under the direction of the Ryan White Program Manager, implements HIV Prevention services. This position performs a wide range of community-based outreach, education, and organizing activities relating to HIV prevention.
Duties And Responsibilities- Provides HIV pre- and post-test counseling services, according to CHN, Federal and State Guidelines.
- Facilitates Evidence Based Interventions (EBIs).
- Coordinates HIV testing services with clinic staff.
- Performs HIV testing.
- Documents Progress Notes on Electronic Health Records.
- Utilize data forms to submit program data.
- Provide wide range of community outreach, education, and general risk reduction.
- Provide other referrals as needed.
- Participates in health fairs, tabling and joint health activities with other community agencies.
- Participate in trainings and support services.
- Supplements pre- and post-test counseling services on-site as needed.
- Maintains strict confidentiality of all HIV related information.
- Ensures linkage to care for every patient diagnosed HIV positive.
- Development of creative HIV prevention counseling techniques compatible with agency mission and resources.
- Compliance with Employee Health Services.
- Performs other related duties, as assigned.
- Associate degree or at least 2 years’ related experience working in case management or HIV prevention field.
