Arista Home Care is growing, and we’re looking for an Intake Manager who’s ready to step into a key leadership role and make a real impact. You’ll lead daily intake and coordination functions, support seamless start-of-care, and serve as a vital link between caregivers, families, MLTCs, and internal teams.

• Lead daily intake and coordination operations
• Keep referrals moving — from authorization through start-of-care
• Support and guide intake coordinators to stay on track and follow through
• Collaborate with MLTCs, caregivers, families, and internal partners
• Help improve workflows to reduce delays and prevent lost referrals
• Ensure documentation and processes meet NYS DOH and payer requirements

• 2+ years of experience in home care intake and/or coordination (LHCSA preferred)
• Strong knowledge of MLTC processes and home care operations
• Excellent communication, organizational, and leadership skills
• Experience working with EMR systems and payer portals
