We are seeking a licensed and experienced Physician Assistant to join our healthcare team in Brooklyn, NY. This role focuses on delivering high-quality primary care services through a telehealth model, providing convenient and accessible care to patients via secure video consultations. The Physician Assistant will play a key role in supporting patients’ ongoing healthcare needs while working in a modern, patient-centered environment.
The ideal candidate is clinically strong, comfortable with technology, and experienced in managing primary care patients remotely. This position is well-suited for a compassionate provider who values continuity of care and effective communication, particularly when working with adult and geriatric patient populations.
Key Responsibilities Telehealth & Primary Care Services- Provide comprehensive primary care services to patients via telehealth platforms
- Conduct video visits to assess patient concerns, review medical histories, and develop treatment plans
- Manage chronic conditions, preventive care, and routine follow-ups in a virtual care setting
- Document patient encounters accurately and thoroughly in the electronic medical record
- Communicate clearly and professionally with patients during video consultations
- Educate patients on diagnoses, treatment plans, medications, and preventive care
- Coordinate care with other healthcare providers as needed to ensure continuity and quality of care
- Utilize the ECW (eClinical
Works) system for documentation, charting, and patient management - Ensure compliance with all applicable clinical guidelines, telehealth regulations, and payer requirements
- Maintain accurate records to support Medicaid and Medicare billing and reporting
- Deliver patient-centered care with a focus on safety, quality, and outcomes
- Stay current with clinical best practices and telehealth standards
- Uphold professional, ethical, and regulatory standards at all times
Required Qualifications
- Current and active New York State Physician Assistant license
- Experience using eClinical
Works (ECW) electronic medical record system - Active Medicaid and Medicare provider numbers
- Strong clinical judgment and ability to work independently in a telehealth setting
- Primary care experience
- Prior telehealth or virtual care experience
- Experience working with geriatric patient populations
- Excellent communication and interpersonal skills
- Comfortable conducting patient visits via video platforms
- Strong organizational skills and attention to detail
- Compassionate, patient-focused approach to care
- Ability to manage time effectively in a structured telehealth schedule
