Responsibilities
  • As a Case Manager, have knowledge and understanding of case management practices and principles.
  • Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psycho educational and/or therapeutic groups.
  • Responsible for communicating with patients and families to keep them involved in treatment plan decisions.
  • Coordinates care with outpatient providers upon admission and discharge.
  • Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
  • As a Therapist, facilitates group, individual, and family therapy sessions.
  • Will be available to provide coverage for other case managers/therapists when absent or as the need arises.
  • Communicate clearly and effectively with clinical team members and Directors.
  • May be assigned supervision responsibilities for those pursuing clinical licensure.
  • Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department.
  • Must be flexible to work at different sites within the Prairie Care metro locations.
Qualifications
  • Candidates must be licensed or actively pursuing clinical licensure in the state of Minnesota. This includes receiving clinical supervision and counting clinical supervision hours. A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or a closely related therapeutic field is required. If your degree is in a closely related field and you are pursuing an LPCC, you must demonstrate that your education meets the coursework requirements of the license, either through your own comparison or an unofficial review with the Minnesota Board of Behavioral Health and Therapy.
  • Two years previous mental health experience strongly preferred.
  • Experience in intensive psychiatric healthcare settings preferred.
  • Experience using electronic health records preferred.
Compensation and Benefits

Compensation for this role ranges from $29- $38 per hour depending on licensure and experience.
