Job SummaryReporting directly to the Practice Manager and Senior Director of Nursing, the Team Nurse is responsible for performing RN/LPN duties in an outpatient primary care or family medicine clinic. This role serves as a key clinical resource for the medical practice and delivers high-quality, direct patient care within a team-based care model. The Team Nurse enhances the safety, effectiveness, and efficiency of patient care by supporting and delegating clinical and administrative responsibilities to Medical Assistant (MA) staff.
This position also plays a critical role in patient education
, care coordination, and supporting smooth transitions between levels of care.
Key Responsibilities
- Provide direct patient care in an outpatient primary care or family medicine setting
- Serve as a clinical resource and support for providers and MA staff
- Delegate and oversee appropriate clinical and administrative functions to MA team members
- Educate and train MA staff and patients on preventive, chronic, and episodic health conditions
- Collaborate with primary care providers to coordinate patient-centered care
- Support transitions between levels of care and promote continuity of care
- Counsel, educate, and support patients in improving overall health status
Skills & Qualifications
- Active Registered Nurse (RN) license (state-specific requirements apply)
- Strong clinical skills in an outpatient or primary care environment
- Ability to work effectively in a team-based care model
- Excellent communication, patient education, and organizational skills
- Ability to support staff training and clinical oversight
- Strong judgment and decision-making abilities
- Must be separated from BILH for 6+ months to be considered
