Primary Responsibilities
- Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors.
- Conduct community outreach and engage community members to understand their health needs.
- Provide individual and group health education and coaching to community members.
- Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services.
- Provide basic health screenings and assessments to identify health risks and provide appropriate interventions.
- Assist in the development of health promotion programs and activities to address community health needs.
- Collect data and maintain accurate records of community health outreach activities and interventions.
- Attend meetings and trainings related to community health promotion and education.
Qualifications:
- High School Diploma required;
Associate’s Degree or Bachelor’s Degree preferred - 1+ year of experience in community outreach, health education, or related field required
- Bilingual (English/Spanish) preferred
Skills and Abilities
- Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
- Demonstrated ability to work effectively and provide advocacy for all populations and communities preferred.
- Ability to work independently and as part of a team.
- Basic computer skills, including Microsoft Office and database management.
- Bilingual skills preferred.
- Full time (40 hours) Monday through Friday, standard business hours
- Hybrid work model
- Covering Fish Center for Women’s Health and Brigham Physician Group at 850 Boylston Street, Brookline
