Your role as a Service Coordinator/Case ManagerAs a Service Coordinator, you play a pivotal role in supporting Oregonians with intellectual and developmental disabilities (I/DD) through impactful person-centered planning and ongoing case management. Your typical day will be filled with excitement as you coordinate with individuals, families, and providers to fulfill essential ODDS tasks, including participating a Needs Assessment (ONA), and the development and monitoring of Individual Support Plans (ISPs).
You'll have the opportunity to visit clients in their homes or community settings to ensure their health and safety, address critical incidents, and facilitate their choices in services while overcoming barriers to support access. Your collaborative efforts with brokerages, providers, schools, and medical teams, along with meticulous documentation will ensure that services are compliant with state regulations and timelines.
