Unit Overview – Interventional Radiology (IR) | RN

Department: Interventional Radiology
Staffing: 7 RNs
Rooms:

• 1 IR suite
• Cath Lab (8th floor) used for vascular procedures
• RNs may work in both IR and Cath Lab on the same day

Average Daily Volume:

• IR: 5–8 procedures/day
• CT prep & IV starts: ~50 patients/day

Required Certifications

• BLS
• ACLS
• PALS (Required for travelers; pediatric experience not required)

Experience Requirements

• Minimum 1 year IR experience
• Must be proficient with IV starts
• First-time travelers accepted
• Critical care experience preferred (bonus, not required)

Common Procedures & Patient Population

Imaging & Diagnostics:

• CT/MRI preps and IV starts (pediatric & adult sedation cases)
• Ultrasound procedures (thyroid needle aspirations)
• Breast biopsies
• Pacemaker patient monitoring

IR Procedures:

• Biopsies (CT- and US-guided, including IR breast biopsies)
• Drain placements: abscesses, pleural effusions, joint collections, paracentesis
• Pain management injections (cervical, thoracic, lumbar spine; shoulder, hip, knee)
• Line placements: PICCs, ports, tunneled catheters, permcaths, chemo ports
• Fistula interrogation

Complex Vascular Cases:

• Embolization (bleeds, fibroids, presurgical tumors)
• Thrombectomies for PE and DVT (INARI, EKOS systems)

RN Responsibilities

• Patient care across all 3 phases : pre-, intra-, and post-procedure
• Assessments and documentation via Radiology Flow sheets
• IV insertions and CT prep
• Patient monitoring and sedation support as needed
• Communication with physicians and mid-level providers regarding plan of care
• Documentation via Allscripts (electronic and some paper charting)

Staffing Model

• 1 RN assigned to:
  • Special procedures (second RN if sedation required)
  • Breast biopsies
  • Ultrasound procedures
  • CT/MRI prep and IV starts

IR Techs:

• CST paired with physician
• IR techs from IR Radiology Department

Scheduling & Shifts

• 12-hour shifts:
  • 7:00a–7:30p
  • 7:00p start times available
• Lunch: 1 hour
• Breaks:
  Two 15-minute breaks
• Travelers provide availability; flexibility required
• No guaranteed block scheduling
• Excessive call-ins may result in termination

Call & Holidays

• No call (weekdays, weekends, or nights)
• 2 permanent RNs staffed overnight
• Holidays covered by permanent staff

Leadership & Support

• Charge Nurse coordinates scheduling and patient flow
• Charge may take patients as needed to support staff
• IR Chair and additional MD in-house during the day; on-call MD after 5p

Floating

• No floating outside IR
• May assist in Cath Lab for IR-specific cases only

Scrubs

• Royal Blue
• Surgical or disposable scrubs permitted in Cath Lab

Orientation

• 3 shifts minimum
• Manager evaluates need for additional support
• Combination of electronic and paper documentation depending on modality
