POSITION SUMMARY
:

The C.N.A assists licensed nursing staff in performing a variety of duties in caring for the residents in the facility. The C.N.A. is expected to have knowledge of policies and procedures for providing bedside and out of bed care.


REPORTS TO:

Unit Director / Charge Nurse


RESPONSIBILITES:

General:

  • Attends beginning of shift report to be aware of the condition of residents on assigned unit.
  • Make preliminary rounds to assess specific assignment.
  • Confers with the Charge Nurse for clarity of assignment, as needed.
  • Keep confidential all knowledge of resident’s problem and/or condition.
  • Administers care in accordance with policies.
  • Escorts residents to clinic and physicians office outside the facility when assigned.
  • Complete accountability records as per facility policy
  • Offer suggestions to the charge nurse that in your opinion will upgrade the quality of care, and participate in overall patient care planning

Keeps the charge Nurse informed:

  • Reports pertinent information, such as but not limited to residents with skin abnormalities, residents with abnormal temperature, residents/visitors complaints, accident/incidents, signs and symptoms of change in patient condition, requests for medication, etc.
  • When resident refuses or is not taking adequate fluids.
  • When the patient refuses to eat or has a poor appetite, or is not following prescribed diet.
  • When patient refuses treatment or any aspect of care which you have been assigned to give.
  • Whenever conditions may be a potential hazard to patients, staff, visitor or employee.
  • Whenever equipment is damaged, malfunctioning or missing
  • When leaving the unit for any reason, such as for supplies, linen, breaks, mealtime, etc.
  • Whenever any problems on the unit requires the service of Housekeeping or Maintenance.

Gives direct care to resident which includes, but is not limited to:

  • Providing personal hygiene-showers, tub baths, sponge baths, etc.
  • Keeping incontinent residents clean and dry.
  • Assisting resident in and out of bed; lifts, turns, and positions patients.
  • Giving oral hygiene and denture care, as needed.
  • Giving back rubs.
  • Assisting residents with toileting, bedpan/urinal use, and empties, clean and sanitize bedpans/urinals.
  • Providing resident privacy and maintain personal dignity of each patient.
  • Assisting with care of nails, hair, etc.
  • Assisting residents in and out of wheelchair to and provide assistance as needed with walkers.
  • Assisting residents in activity according to tolerance level and M.D. order and accountability record.
  • Insuring that residents are cleaned of spills after meals.
  • Empting urine drainage bag at the end of each shift.

Prepares patients for meals:

  • Distributes bibs as needed
  • Serves trays and return them to the food truck after meals
  • Feeds and/or assists residents who need help.
  • Serves between meal nourishments and provides fresh water to patients.
  • Checks to insure that all residents eating have been served a tray.
  • Wipes tables clean after meals.
  • Maintains resident care units in a neat and orderly condition:
  • Changes and makes beds.
  • Clears bedside unit of food items and clutter, and insures that contents of the bedside unit are clean.
  • Keep residents closet and drawers neat with only the property of the specific patient.
  • Strips resident unit upon transfer, hospitalization, discharge, or expiration, in order that the unit may be cleaned by Housekeeping staff. Checks and packs patients’ personal property for disposition in either case as listed above.

As directed by the Charge Nurse, performs selected procedures and simple treatments:

  • Takes and records vital signs, weight and height.
  • Measures and records fluid intake and output.
  • Collects routine specimen, such as urine, stool, sputum, etc.
  • Administers enemas
  • Assist with rehabilitative measures, such as, walking, range of motion exercises, transfers, etc.
  • Transports and accompanies patients to various areas in the Nursing Home.
  • Maintains bowel and bladder retraining program for specified residents.
  • Provides post-mortem care, when required
