-Experience with Care Coordination, Utilization review and discharge planning Computer literacy.

Education: Required

Education:

-Associates Degree (ASN, ADN) from an accreditated Nursing Program.

Required Certifications & Licensure:

- RN Licensed as a Registered Nurse in the State of New York.

-PRI and Screen certification within three months of employment.

-Primary Source Verification of License and Registration of Registered Professional Nurse from NYS Department of Education (NYSED).

-BLS. Preferred Certifications & Licensure:

-Case Management Certification.

Schedule Notes:
• 5 years minimum of clinical experience as a Registered Nurse.
