Compensation:
Pay Range: $92,000-$94,000 Per Year

Position Summary:

• Provide nursing care and support to program participants, including health assessments, treatment planning, and coordination of medical services.

• Collaborate with the ACT team to ensure participants receive integrated physical and mental health care.

Responsibilities:

• Conduct nursing assessments and develop individualized care plans

• Implement and evaluate nursing care

• Coordinate health services with the ACT team and other providers

• Lead group sessions on health, wellness, and recovery

• Educate staff on medical care and medication procedures

• Advocate for participants to access medical care and community supports

• Supervise LPNs on medication management

• Ensure participants have primary care providers and receive regular exams

• Administer medications, vaccines, and perform basic procedures

• Maintain accurate health records and documentation

• Provide culturally competent care and crisis intervention as needed

Qualifications:

• Licensed and registered as a professional nurse in NYS

• BSN preferred

• Valid driver's license

• Bilingual (Spanish) desirable

• Experience with serious mental illness, forensic populations, co-occurring disorders, or HIV/AIDS preferred

• New grads welcome
