Role: Advanced Practitioner – Adult Neurodiversity Team

Location: Bromsgrove

Salary: £32,020 – £36,298 (up to £38,296 with AMHP) + £3,000 Market Forces payment

About the Role

You will promote the well‑being of adults and carers, facilitating independence and access to community resources. Your responsibilities include:

  • Engaging in meaningful conversations with adults and carers to assess needs and develop personalized support plans.
  • Collaborating with professionals and agencies to provide multi‑agency support, enabling individuals to live independently.
  • Ensuring compliance with statutory requirements, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Participating in safeguarding procedures and interventions, utilising the Making Safeguarding Personal approach.
  • Representing the council in court proceedings and advocating for the welfare of service users.
  • Conducting assessments and support planning using a strength‑based approach.
  • Collaborating with stakeholders to create sustainable care packages and prevent the need for statutory services.
  • Maintaining accurate records and engaging in reflective practice and supervision.
  • Promoting the welfare of children and reporting safeguarding concerns as necessary.
Requirements

Experience:

  • Proven experience in social work, including assessment and support planning for individuals and caregivers.
  • Experience in adult social care, especially with older adults and those with physical disabilities.
  • Track record of involving users and caregivers in their care arrangements.
  • Extensive experience collaborating with health and social care agencies and professionals.
  • Proficiency in utilising community resources and networks to develop sustainable care plans.
  • Evidence of Continuous Professional Development.

Knowledge, Skills & Abilities:

  • Understanding of social work theories and practices.
  • Familiarity with legislation pertaining to adults and caregivers.
  • Proficiency in strength‑based practice.
  • Clear and effective verbal and written communication.
  • Strong interpersonal skills.
  • Effective organisational and prioritisation abilities.
  • Capacity to collaborate within multidisciplinary teams and establish relationships with external agencies.
  • Competence in assessment and support planning for adults and caregivers.
  • Ability in partnership working and negotiation.

Qualifications:

  • Professional qualification in social work.
  • Current registration with Social Work England.
  • Relevant post‑qualifying certifications.
  • Level 1 qualification in Essential Digital Skills or proficiency in Microsoft Office.

Additional Information:

  • The role may require travel throughout the county, including areas with limited public transport.
  • Reasonable adjustments will be considered as required by the Equality Act.
  • This position is subject to an enhanced Criminal Records Check.
  • Strong English language proficiency is essential.
