Position Summary

The Nurse Practitioner provides primary medical care and treatment to an assigned panel of BNHC patients in accordance with professional standards and established organizational policies.

Key Duties & Responsibilities
  • Deliver comprehensive primary care, including well exams, immunizations, and sick visits.
  • Provide education and management focused on health promotion, illness prevention, and screening.
  • Develop patient-centered care plans addressing physical, emotional, and social needs in collaboration with patients and families.
  • Coordinate care and case management across the multidisciplinary health team to ensure continuity of care.
  • Initiate specialty referrals and in-house referrals for both medical and non-medical conditions.
  • Educate patients on the importance of regular well visits and preventive care.
  • Maintain a culturally and linguistically sensitive practice, using interpreters when needed.
  • Collaborate with caregivers to promote both physical and mental health.
  • Foster strong relationships with providers, administration, and staff to ensure high-quality care delivery.
  • Participate in policy and protocol development within the department.
Quality Standards
  • Attend at least two-thirds of provider and staff meetings annually.
  • Participate in at least one Performance Improvement team or community outreach project per year, or complete an approved independent quality project.
  • Submit electronic encounters and documentation by the next business day.
  • Maintain 90%+ compliance with electronic problem lists, medication lists, prescribing, and pain assessment standards.
  • Comply with EHR/Meaningful Use requirements.
  • Support the Patient Care Committees efforts to monitor and improve quality of care.
  • Adhere to all regulatory and compliance standards.
Communication Standards
  • Demonstrate sensitivity to patient, family, and staff communication both verbal and non-verbal.
  • Protect patient privacy and dignity in all written and oral communications.
Problem Resolution
  • Exercise sound judgment in resolving clinical and non-clinical issues.
  • Act independently and make decisions confidently and appropriately.
  • Anticipate potential problems and participate in proactive problem-solving.
Interpersonal Skills
  • Display tact, patience, and discretion in all interactions.
  • Remain calm and professional in high-pressure situations.
Qualifications
  • Current Massachusetts RN licensure in expanded role; eligible for certification as a Primary Care Practitioner.
  • Federal and State licensure for prescriptive authority.
  • Experience working with multidisciplinary teams (e.g., social workers, case managers).
  • Fluency in Spanish, Portuguese, French, or Haitian Creole strongly preferred.
  • Previous ambulatory care or public health experience preferred.
  • Proficiency in electronic medical records required.
Working Conditions
  • Fast-paced community health center located in a downtown district.
  • Occasional long hours may be required.
  • Potential exposure to patient blood and body fluids.
Physical Requirements
  • Lift up to 50 lbs (equipment, boxes, or patients).
  • Frequent standing and walking required.
  • Adequate vision and hearing for routine patient care and conversation.
Travel & Work Hours
  • Occasional statewide or national travel for CME/conferences.
  • Includes evening and weekend hours as needed.

Actual compensation is based on multiple factors such as relevant experience, education, and training.
