Nurse (RN)/Case Manager to manage and support our Adult Family/Foster to makesure the our programs runs smoothly.

Job Title: MDS Nurse Assessment Coordinator for AFC in Brockton

Location: Brockton, MA

Compensation: $100 per MDS Nurse Assessment and $50 per visit

Employment Type: Full-Time or Part-Time

We are seeking a caring and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Initial Assessment / Admission Nurse. In thisrole, you will complete in-home evaluations for individuals applying to the AFC program, reviewing their needs and confirming eligibility. This opportunity isideal for nurses with experience in geriatrics, home health, or community-basedcare who are committed to supporting individuals and enhancing quality of life.

• Conduct comprehensive in-home assessments for new AFC applicants.
• Evaluate medical, functional, and psychosocial conditions to determine eligibility.
• Partner with caregivers,families, and healthcare professionals to design customized care plans.
• Provide education to participants and families regarding AFC services, expectations, and program policies.
• Ensure compliance with allstate regulations and AFC program requirements.
• Maintain accurate, up-to-date documentation of assessments, care plans, and follow-up activities.

• Demonstrated ability to coach, train and provide staff development
• Proficiency in Microsoft (Word, Excel and PowerPoint)
• MS or Bachelor’sdegree in Nursing
• Must befully licensed RN by Mass Board of Registration
• At least five years of previous home health care experience
