Shift: Days, evenings, weekends, and overnight or on call as needed
High Point is expanding mental health services in the Brockton community, providing 24/7/365 crisis intervention services focused on acute mental health crises. The Mobile Crisis Intervention (MCI) agency will support evaluation, safety assessments, and referrals to inpatient, outpatient, and dual diagnosis services for adults and youth.
Team includes licensed clinicians, Master’s level clinicians, certified peer specialists, patient resource navigators, nursing staff, nurse practitioners, psychiatrists, medical physicians, and psychologists, offering a continuum of care for mental health, substance use, and dual diagnosis.
Duties and Responsibilities- Responsible for linking patients with aftercare services prior to discharge and aiding the patient with access to outpatient services when appropriate
- Evaluating with patient what has been successful for them in the past and what were barriers to avoiding re‑hospitalization so they can be more independent in the future when supporting service referrals
- Provide post evaluation follow up face to face or phone support as clinically indicated
- Monitoring patients onsite during disposition planning, or boarding at community location
- Respond to emergency phone calls offering phone triage and area resources as appropriate
- Verify patient insurance, including benefits, eligibility and service options
- Triage calls, evaluation requests and report back to the program director/supervisor on site or AOC
- Complete safety screens for community-based evaluation requests
- Execute bed searches for clients awaiting placement
- Escalate complicated cases to the appropriate supervisor to advocate for the needed resources/supports
- Assist with daily protocols such as the EPIA process
- Update tracking of pending and boarding cases for evaluation requests
- Assist the call center as needs arise or as requested by a supervisor
- Documentation of interventions and outcomes in electronic health record
- Attend mandatory trainings
- Responsible for contacting the insurance company for authorization, as needed
- Utilize standards precautions at all times
- Demonstrates behaviors that recognize the rights of patients as defined by the patient’s rights
- Other duties as assigned by the Clinical Supervisor and/or Program Director.
- Bachelors degree with major preferred in psychology, social work, or social sciences
- At least two years of experience preferred working in behavioral health (inpatient setting, outpatient community mental health, or emergency room)
- Ability to work in a fast-paced environment
