What you will do:- Assess the health and functional status of members.
- Develop community-based care plans and related service packages.
- Participate in the development of a member's Individual Care Plan (ICP).
- Arrange, coordinate, and monitor the provision of appropriate community services.
- Promote the independent functioning of members.
- Participate in the determination of a member's need for institutional and community long term care services.
- Your passion for helping others and desire to make a difference.
- Bachelor's degree in social work or related field.
- Minimum of two years' professional experience in the care of persons 65 and older.
- Current Social Work license (LSW or higher) preferred.
- Working knowledge of the social, emotional, and physical processes of aging.
- Ability to gain and hold the confidence of clients and their families.
- Ability to obtain information of a personal nature through interviews.
- Draw appropriate conclusions and make recommendations.
- Knowledge and skill necessary to work successfully within an interdisciplinary team; ability to express clearly, both orally and in writing.
- Ability to work with an enthusiastic team of like-minded individuals.
