Summary:

• Provide medical-surgical pediatric and obstetrical case management services to Brockton Hospital patients.
• Ensure continuity of care for patients under specific physician groups.
• Assess, coordinate, and implement plans for quality, cost-effective care for patients in the pre-admission, emergency, acute, and ambulatory settings.
• Proactively identify high-risk patients for timely case management interventions.
• Collaborate with attending physicians, PCPs, and other hospital clinical and administrative staff to develop and implement effective discharge plans.
• Monitor appropriate use of hospital resources and facilitate the delivery of services.
• Use accepted criteria and guidelines to conduct review of inpatient admissions.
• Serve as the primary liaison between PCP/attending physician, patient/family, payer-based case managers, and appropriate community-based providers.

• Minimum of five years of clinical experience in an acute care facility.
• Current license from the Commonwealth of Massachusetts to practice as a Registered Nurse.
• Prior experience in case management, utilization review, or discharge planning required.
• Education:
  
  High School Diploma or equivalent required. Bachelor’s Degree required; may be waived with experience commensurate with job expectations. Master’s Degree preferred.
