DESCRIPTION
Provide supportive assistance, personal care & training to consumers in their daily living, social, recreational, day services and community activities, in accordance with the goals & requirements prescribed in each consumer's behavior plan and in a manner that facilitates cross‑team collaboration, cooperation and service integration across work unit, department and organizational boundaries. Provide support & comfort to people with developmental and other disabilities, assisting them to lead self‑directed lives, achieve their personal goals and contribute to their community;
and encourage attitudes and behaviors that enhance inclusion in their
communities.
Maintain effective audio/visual discrimination and perception for reading and writing, completing standardized forms, operating assigned equipment/vehicles, basic computer skills – knowledge and ability, which allow the use of computers and related technology such as iPads, tablets and smartphones. Maintain mental capacity and reasoning ability, which allows the capability of making sound decisions based on appropriate judgement especially in situations that pose risk to the health, safety and well‑being of persons served as well as dealing with abstract and concrete variables, developing and preparing clear and meaningful reports and demonstrate strong interpersonal skills including ability to effectively collaborate with others.
WORKSCHEDULE
This is a full‑time position with a mandatory 24 hours one day per week with a minimum of 35 hours: 6 days of on‑call shifting with a 2 hour prior notice for on‑calls. There is no set schedule and will float between group homes. Hours may vary.
