We're looking for an enthusiastic and adaptable Community Nurse to join our Thanet Community Nursing Team. You'll be part of a supportive, close‑knit group who share our Trust CARE values and are dedicated to delivering high‑quality nursing care to the local community.

Community Adult Nursing teams in Kent are aligned with Primary Care Network footprints, enabling more integrated and innovative ways of working. This approach ensures care is holistic, needs‑led and responsive to the people we serve. Working closely with primary care colleagues, you'll help shape how we deliver community‑based care now and in the future.

Thanet offers a diverse patient population with a wide range of complex needs. You'll provide proactive, patient‑focused care that supports people to remain safe and well 'll build meaningful relationships, use your clinical skills to manage long‑term and acute conditions, and contribute to improving outcomes across the area.

Main duties of the job

As a Community Nurse, you'll support the nursing needs of housebound patients across Thanet, enabling and empowering people to stay safe and well in their own homes or chosen environment. You'll play a key role in helping patients manage long‑term conditions, maximise independence and avoid unnecessary hospital admissions, as well as supporting quicker, safer discharges when hospital care is needed.

You'll deliver a wide range of community‑based interventions, including wound management, palliative and end‑of‑life care, symptom control, catheter care, venepuncture and the administration of injectable medications.

You'll be a vital member of our multidisciplinary team, managing patients with complex needs in the community. The role offers autonomy and the freedom to plan care in a way that works for you and your patients, with time to develop your assessment and treatment skills. You'll have access to excellent supervision, clinical leadership and a nurturing preceptorship programme, alongside ongoing development opportunities to support your growth.
