What you will do:

• Visit patients in their homes or community sites to conduct thorough assessments and provide effective neurorehabilitation and/or neurodisability management.
• Collaborate closely with a multidisciplinary team to develop and implement personalized care plans.
• Monitor patient progress and adjust treatments as necessary to maximize recovery and outcomes.
• Educate and support patients and their families, empowering them to enhance their health and wellbeing.
