RN Case Manager

Position: RN Case Manager - Full Time

** 401K The registered nurse/case manager plans, organizes, directs, and provides hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

* Minimum Qualifications:

** Minimum of two years of experience as a Registered Nurse in a clinical care, home health or hospice setting preferred.

Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

* Key Responsibilities:

** Monitor assigned cases to ensure compliance with requirements of third-party payer. Perform on-call responsibilities and provide on-call service to patients/families as assigned. Perform other nursing duties as assigned by management to support department goals and initiatives.
