Overview
Job Summary:
We are currently looking for an experienced, enthusiastic, flexible and reliable Primary Care Practice Nurse to join our friendly and supportive team.
We are looking for a Nurse who has previous experience of working in primary care as a Practice Nurse and unfortunately, we are unable to accept any applications who do not have this experience.
Hours: We are looking for 21 hours a week but will consider up to 37.5 hours a week, within the Practice opening times of 07.30-18:30 (Monday-Friday) with enhanced hours on a Thursday evening until 8:00pm.
Salary depending on experience
An exciting opportunity has arisen for an experienced Practice Nurse to join our established and collaborative Nursing team at our forward thinking and supportive practice. We have a dynamic team of nurses, healthcare assistants and phlebotomist providing chronic disease management and enhanced services to our 14,500 patient list.
We Are Looking For:
- Proven experience in a Primary Care setting
- Competence in chronic disease management, specifically in Diabetes or Asthma (minimum requirement)
- A commitment to ongoing professional development and willingness to undertake further training as needed
- A team player who is proactive and adaptable to the evolving challenges of Primary Care

ResponsibilitiesMain duties of the job
- Provide and maintain a high standard of nursing care for patients, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population.
- Day to day primary care nursing such as phlebotomy, ECGs, wound care, adult and child immunisations, B12 injections, BP monitoring;
Enhanced Practice Nurse skills in chronic disease management, cervical screening and compression dressings. - Safeguard and promote the welfare of children and vulnerable adults in line with NHS Trust safeguarding policies and procedures; attend mandatory safeguarding training.
- Document contemporaneous consultations according to NMC guidelines using EMIS clinical system; maintain accurate clinical records in conjunction with legislation; ensure infection control guidelines are maintained.
- Provide vaccination and travel medicine services; administer vaccines under patient group directions; manage long-term conditions and chronic disease registers.
- Prioritise health issues, deliver opportunistic health promotion, and support safeguarding matters in line with local and national policies.
- Perform a holistic assessment of patients attending for cervical cytology (if carried out), recognise and refer mental health needs, and respond to clinical emergencies as required.
- Identify personal development and training needs in conjunction with the Nurse manager and deputy Practice Manager; participate in clinical supervision and annual appraisals; undertake mandatory training and induction programs.
- Be familiar with emergency procedures; contribute to clinical governance; attend team meetings and maintain an informed interest in practice objectives.
Knowledge and Skills
- Essential:
Previous nursing skills in primary care (ECGs, phlebotomy, wound care, immunisations, etc.); excellent communication; cervical screening; chronic disease management; compression dressings. - Desirable:
Knowledge of NHS primary care issues; travel vaccinations; contraception reviews.
Qualifications
- Essential:
Registered General Nurse (RGN) with current PIN; evidence of continued professional development; experience in chronic disease management; vaccination/immunisation training. - Desirable:
Prescribing; COPD.
Personal Qualities
- Enthusiastic about developing nursing skills; maintain confidentiality; ability to work with minimal supervision; team player; conscientious and adaptable under pressure; good communication and initiative.
Other
- Essential:
Ability to deliver care within patients’ homes for housebound chronic disease checks and vaccinations (flu, pneumococcal, COVID, Shingles). - Desirable:
Flexible working hours; car driver/clean licence/business insurance.
