Diabetes Practice Nurse - General Practice

Overview

A rare and exciting opportunity to join Greenway Community Practice as a part‑time Diabetes Specialist Nurse. The role collaborates closely with the Clinical Nurse Lead and GP Diabetes Lead to provide specialist diabetes clinics and support long‑term condition management.

• Carry out structured diabetes reviews for patients with Type 1, Type 2 and pre‑diabetes in line with NICE and local guidelines.
• Monitor and assess HbA1c, blood glucose and relevant investigations, and review cardiovascular risk factors.
• Provide education and advice on lifestyle modification, diet, exercise and self‑management.
• Support initiation, adjustment and monitoring of diabetes treatments, including oral medications and injectable therapies (subject to training and competence).
• Identify patients requiring escalation of care and liaise with GPs, diabetes specialist nurses and secondary care teams as appropriate.
• Maintain accurate documentation, contribute to QOF, audits and chronic disease registers.
• Deliver routine treatment room services, including wound care, injections, immunisations and venepuncture, in accordance with practice protocols.
• Provide general and specific health screening and health education to the practice population with referral to GPs as necessary.
• Assist in minor surgical procedures and clinical procedures where appropriate.

• Adult Nursing qualification.
• Minimum 2 years post‑registration experience.
• 1+ years experience working in primary care.
• Qualified to Degree level with holding qualifications in diabetes.
• Experience in nurse‑led management of chronic conditions.
• Experience in implementing protocols and clinical guidelines.

• Mentor/ teaching qualification – experience of teaching or mentoring.
• Experience and interest in education within a primary care setting.
• Independent and/or supplementary nurse prescribing qualification.
• Community nursing specialist qualification or equivalent.
