Overview
We are currently looking for an experienced, enthusiastic, flexible and reliable Primary Care Practice Nurse to join our friendly and supportive team. We require a Nurse with previous experience in primary care as a Practice Nurse.
Hours: 21 hours a week (will consider up to 37.5 hours) within Practice opening times of 07:30–18:30 (Monday–Friday) with enhanced hours on a Thursday evening until 20:00. Salary depending on experience.
An exciting opportunity has arisen to join our established and collaborative Nursing team at our forward-thinking and supportive practice. We operate with a dynamic team of nurses, healthcare assistants and phlebotomists, providing chronic disease management and enhanced services for our 14,500 patient list.
Responsibilities- Provide and maintain a high standard of nursing care as part of the practice multidisciplinary team, delivering care within scope of practice to the entitled patient population.
- Day-to-day primary care nursing tasks including phlebotomy, ECGs, wound care, adult and child immunisations, B12 injections, BP monitoring; advanced skills in chronic disease management, cervical screening and compression dressings.
- Safeguard and promote the welfare of children and vulnerable adults in line with NHS safeguarding policies; attend mandatory safeguarding training.
- Document contemporaneous consultations according to NMC guidelines using EMIS; maintain accurate clinical records; manage referrals and pathology services as needed.
- Provide vaccination and travel medicine services; administer vaccines under patient group directions; manage vaccination schedules.
- Support the clinical team with safeguarding matters and health promotion; undertake cervical cytology assessments where applicable.
- Prioritise health issues, manage long-term condition risk, and deliver chronic disease clinics; maintain chronic disease registers.
- Support and supervise junior nursing staff; participate in local initiatives to enhance service delivery and patient care.
- Contribute to clinical governance, audit participation, training, and ongoing professional development; attend team meetings and formal appraisals.
- Develop, implement, and embed health promotion and well-being programmes; evaluate treatment plans for chronic disease patients.
- Assist GPs with minor surgery when required; chaperone patients as needed; respond to clinical emergencies; liaise with external services to support patients.
- Monitor safe storage and disposal of medicines; ensure appropriate use of read codes and templates; comply with infection control procedures.
- Identify personal development needs in conjunction with Nurse Manager and Deputy Practice Manager; participate in clinical supervision and annual appraisals; undertake mandatory training.
- Be familiar with emergency procedures and contribute to clinical governance.
- Essential:
Primary Care nursing experience; current RGN with PIN; evidence of CPD; vaccination/immunisation training; ability to deliver care in patients’ homes for housebound checks and vaccinations. - Desirable:
Knowledge of NHS primary care issues; prescribing; COPD; EMIS experience; willingness to gain further nursing skills. - Essential qualities: ability to work with confidentiality, good communication, teamwork, initiative, resilience under pressure, and effective time management.
- Experience:
minimum 2 years in primary care; experience dealing with patients and the public.
- Essential:
Registered General Nurse (RGN); current PIN; evidence of CPD; chronic disease management experience; vaccinations/immunisation training. - Desirable:
Prescribing qualification; COPD experience.
- Essential:
Ability to deliver care within patients’ homes for housebound checks and vaccinations (flu, pneumococcal, COVID, Shingles). - Desirable:
Flexible working hours; car driver with clean licence and business insurance.
- Disclosure and Barring Service (DBS) checks as applied to the Rehabilitation of Offenders Act 1974; current UK professional registration required.
- Adherence to NMC Code of Conduct and practice policies; compliance with risk, quality, and confidentiality standards.
