Conduct home visits to assess patients' health needs and provide care

Administer medications and treatments in a home setting

Manage chronic conditions and provide ongoing monitoring

Offer wound care, injections, vaccinations

Provide support and education to patients and their families

Act as a liaison between patients and the wider healthcare team

Coordinate care and services for patients within the district

Advocate for patients' healthcare needs and access to resources

Contribute to public health initiatives and health promotion programmes

Keep accurate patient records and documentation

Respond to emergencies and provide urgent care when necessary
