Community Children's Nurse - Continuing Care Team
We are looking for a passionate Childrens Nurse to join our Continuing Care Team! This is a part time position for 30 hours per week.
The Lifetime Service is a Community Children's Nursing (CCN) and Psychology Service for Children and young people with non-malignant, life limiting and life‑threatening conditions.
The team is based in Kingswood and provides a service across 3 ICB areas, supporting children aged 0‑18 and their families, in Bristol, North Somerset and South Gloucestershire (BNSSG). The service has a Continuing Care Team (CCT) which provides unregistered trained support workers to work with children and young people who meet the children's continuing care framework.
Main duties of the jobTo work within the Continuing care team (CCT) to support children and their families who meet the Children's Continuing care criteria and to work with children with complex health care needs and palliative care in the community. To be responsible for the line management of Community Children's nurses and Lifetime support workers (LSW). To facilitate early discharge from acute settings and fully integrate into the community.
To deputise for the Team leader of CCT as required.
To line manage, to be involved in the recruitment, induction, training and preceptorship of junior members of staff. Creating, delivering and evaluating training programmes, specific to a child's, within a multi‑agency environment. To be involved in the rota for LSW ensuring that commissioned care needs are met. To ensure that care is delivered to agreed standards and within the policy framework of Sirona Care and Health and NMC guidelines.

Person Specification Qualifications Skills, experience and knowledge- Knowledge of professional agenda and NMC standards
- Ability to support and challenge in a supportive way
- Understanding of systems and processes in relation to safe service delivery
- Experience of working as part of a multi‑disciplinary team
- Understanding of Equality and Diversity
- Ability to work in a changing environment
- Children's Continuing Care experience
- Clinical experience encompassing Special Schools, Children's Community Nursing, Complex Care and Palliative Care
- Experience of complex rota's to meet service and staffing demands.
- Relevant community experience i.e. Specialist Practitioner CCN or SCPHN

£38,682 to £46,580 a year per annum, pro rata
