About the Role
There are two Senior Dementia Practitioner roles available, 1x 0.6 wte and 1x 0.8 wte. Both roles are based within the post diagnostic team and will work from Brookland Hall. These city‑wide roles will focus on delivering high quality, individualised support to service users and carers, encompassing a wide range of 1:1 and group interventions. The post holders will work collaboratively with the Psychology team to deliver bespoke interventions for complex service users and carers.
Key Responsibilities- Work alongside a Clinical Support Manager to support the development and implementation of the services Preventing Well agenda, engaging with stakeholders and others in defining and delivering this pathway.
- Engage in initiatives to promote good brain health, maintain function and independence, and reduce risks for developing dementia.
- Set up and run groups, perform goal setting, and implement outcome measures.
- Apply cognitive rehabilitation to promote independence and adaptation to life changes.
- Provide clinical supervision of support staff, demonstrating leadership to support own and team development.
- Contribute to the services access‑point rota, answering and responding to clinical calls on a regular basis.
- Experience setting up and running groups.
- Goal‑setting skills and knowledge of the implementation of outcome measures.
- Desirable experience using cognitive rehabilitation.
- Experience providing clinical supervision of support staff.
