Job DescriptionYou will lead a small team of First Contact Practitioners and work directly with patients across our Primary Care network of practices in South Bristol.
Required qualifications:
advanced practice qualification (post‑grad diploma, degree or Masters if qualified after 2020) and prescribing qualification or on‑track to obtain within twelve months.
- Independently triage, assess, evaluate, diagnose complex disorders and initiate management plans.
- Justify clinical referrals to specialist services and manage referral pathways.
- Recognise red flags, serious pathology and potential underlying non‑MSK disease.
- Initiate medication prescribing and administration where competent.
- Maintain accurate patient records and work collaboratively with multidisciplinary teams.
- Lead service development, audit, research and continuous improvement.
- Provide supervision and support for the FCP team.
- Lead service improvement projects and implement evidence‑based standards.
- Plan and implement PCN/practice developments.
- Promote a culture of learning, facilitate training and provide supervision.
- Initiate and contribute to research and audits to improve patient management.
- Interpret findings and disseminate internally.
- Establish strong relationships with physiotherapists, staff and GP leads.
- Encourage good clinical practice and support care coordinators.
- Undergraduate and Masters degrees in Physiotherapy.
- Registration with the Health and Care Professions Council.
- Member of the Chartered Society of Physiotherapy.
- Experience of clinical supervision, leadership and management.
- Excellent IT skills, EMIS Web experience.
- Managing and supervising staff.
- Clinical leadership and project management in health services.
- Ability to recognise priorities, refer appropriately and adhere to policies.
- Involvement of patients in medication decisions.
- Use of GP clinical system EMIS Web.
- Non‑medical prescribing qualification.
- Training in specialist procedures such as joint injections.
Submission to the Disclosure and Barring Service is required under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
