Position: Recovery Navigator (Later Life)

Main duties of the job

This is a support role helping older adults often with complex needs, like mental health, housing, substance abuse, fragility and social isolation, connect with services and build independence. The Recovery Navigator acts as a key link, between clinical care and community support to improve well-being and achieve personal goals, focusing on the unique challenges of aging.

Job responsibilities

Working with families, carers and friends and be working in a multidisciplinary team.

Building therapeutic relationships with service users.

Support a caseload of severe and enduring mental health needs of service users.

An excellent communicator.

Receiving management and clinical supervision, together with group reflective practice.

Person Specification Experience
  • Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions)
  • Two years proven experience working with clients over the age of 50 in a community setting.
  • Proven experience of carrying out needs and/or risk assessments and the support planning process
  • Experience of working with a number of individuals with competing needs and priorities
  • A year s experience
